ABSTRACT

La céramique est utilisée depuis longtemps en dentisterie. Elle est considérée comme le matériau le plus esthétique. Les céramiques sont devenues de plus en plus populaires comme matériaux de restauration en raison de leurs esthétique, leurs inertie et leurs biocompatibilité. De nombreux différents types de systèmes céramiques ont été introduits ces dernières années qui pourraient être utilisés pour des restaurations indirectes. Ils peuvent être déroutants pour le clinicien de les connaître et de faire le choix par la suite. Il est important de les comprendre de façon simple. Leur compréhension pourrait être facile en se basant sur le développement historique, la structure chimique et leurs applications. Cependant, les problèmes majeurs de la céramique sont leur fragilité et leur susceptibilité aux fractures [Lawn etal. 2001]1, [Griggs., 2007]2. Les couts financiers pour le développement d'une céramique résistante aux fractures et esthétique sont élevés : la couronne européenne et le marché FDP avaient approché 2 milliards de dollars en 2007 [Millennium ResearchGroup ; 2007]3 ;la couronne mondiale et le marchéFDP était estimé à 25 milliards de dollars en 2010 et à plus de 30 milliards de dollars en 2015 [Palmer., 2010]4. Cet article fournit un aperçu de la céramique dentaire qui est présenté d'une manière simplificatrice permettant une compréhension de leur développement

ABSTRACT

Ce cas clinique décrit la prise en charge d'un édentement maxillaire unilatéral de grande étendue par une prothèse composite stabilisée sur implants. Dans cette situation clinique défavorable et lorsque la solution par prothèse fixée sur implants ne peut être retenue pour des raisons locales ou générales, la prothèse amovible partielle métallique (PAPM) reste l'alternative de choix. Elle peut être stabilisée efficacement par des implants. En effet, l'utilisation de ces derniers comme support de moyens de rétention complémentaires est d'un apport considérable. Ainsi, la PAPM stabilisée sur implants permet de modifier l'architecture de l'édentement, d'optimiser l'équilibre prothétique, de rallier exigences esthétiques et fonctionnelles assurant alors une meilleure intégration prothétique

ABSTRACT

Se presentan los casos clínicos de 3 adultos con dientes extraídos, atendidos en la consulta de prótesis dental del Policlínico Docente 30 de Noviembre" de Santiago de Cuba, a quienes se les colocó prótesis inmediata como única solución ante el deterioro bucal que presentaban. Con el proceder realizado se logró la recuperación estética de los pacientes, así como la reinserción en las actividades sociales y laborales, evitando así las afectaciones psíquicas que suelen aparecer


The case reports of 3 adults with extracted teeth are presented. They were assisted in the dental prosthesis service of 30 de Noviembre Teaching Polyclinic in Santiago de Cuba to whom immediate prothesis were placed as the only solution due to the oral deterioration that presented. With this procedure the cosmetics of the patients was recovered and their reintegration to the social and labor activities was achieved, with which the psychic disorders that usually appear were avoided

ABSTRACT

OBJETIVO: o objetivo deste trabalho foi relatar um caso clínico de periodontite crônica localizada, no qual foi indicada exodontia dos elementos comprometidas e os mesmos foram aproveitados para confecção de uma prótese provisória imediata. DESCRIÇÃO DO CASO: Paciente E.A.S.N, gênero masculino, atendido na clínica da FacDelta - UNIME Salvador, foi diagnóstico com periodontite crônica severa localizada e planejado exodontia das unidades comprometidas e reabilitação com prótese provisória imediata, utilizando os próprios dentes recém-extraídos. Após secção na altura do terço médio da raiz e remoção da polpa, os elementos foram selados com resina composta fotopolimerizada, seguindo o protocolo de restauração adesiva. Posteriormente, um fragmento de foi ortodôntico pré-curvado foi fixado nos dentes pilares assim como os dentes extraídos foram fixados ao foi utilizando resina composta. Oclusão foi ajustada para não haver contatos cêntricos e excêntricos, realizado acabamento e polimento e instrução de higiene oral e sobre a cuidados com a mastigação na região. CONCLUSÃO: Prótese provisória imediata utilizando os próprios dentes do paciente recém-extraídos configurou uma opção reabilitadora viável para este caso devido às restrições de tempo e financeira do paciente, por ser uma técnica conservadora e na obtenção da sua satisfação


OBJECTIVE: The objective of this study was to report a case of localized chronic periodontitis, and the extraction of committed elements was indicated and they were utilized in the immediate provisional prosthesis. CASE DESCRIPTION: Patient E.A.S.N, male, attended in the FacDelta - UNIME Salvador clinic, he was diagnosed with located severe chronic periodontitis and planned extraction of the affected units and rehabilitation with immediate provisional prosthesis using their own extracted teeth. After had been sectioned in the middle third of the height of the root and removed the pulp, the elements were sealed with light-cured composite resin, following the adhesive restoration protocol. Subsequently, a fragment of pre-curved orthodontic wire was fixed in abutment teeth and extracted teeth were fxed to wire using composite resin. Occlusion was adjusted in order to avoid contacts, it was done the fnishing and polishing and the patient was instructed about oral hygiene and the avoidance with chewing in the region. CONCLUSION: immediate provisional prosthesis using their own extracted patient's teeth was a viable rehabilitative option for this case due to patient's time and fnancial constraints; it was a conservative technique and got his satisfaction

ABSTRACT

Existe poca información objetiva acerca del perfil de emergencia en implantes dentales que aborde el tema de manera global e integral. Sin embargo, varios aspectos que involucran el perfil de emergencia de una restauración han sido ampliamente estudiados y documentados en forma separada. Habiéndose analizado en la primera parte de este artículo la relación entre los aspectos biológicos, quirúrgicos y los diferentes tipos de implantes disponibles, en esta segunda parte se amplía la revisión sobre el manejo quirúrgico y clínico, con el fin de lograr un enfoque objetivo para la comprensión del perfil de emergencia en restauraciones sobre implantes dentales.

ABSTRACT

FUNDAMENTO: la avulsión dentaria es la salida completa del diente de su alveolo, el mejor tratamiento para un niño es la reimplantación de su propio diente, pero la terapéutica ofrece grandes retos al profesional que dependen de múltiples factores. OBJETIVO: demostrar que la rehabilitación por prótesis inmediata, es una alternativa ante el fracaso de un reimplante dentario. CASO CLÍNICO: paciente masculino, de doce años de edad, que acudió con su padre a la consulta de prótesis estomatológica del Policlínico de atención a combatientes de los servicios médicos del MININT de Camagüey, porque hace dos años y medio le fue reimplantado el diente antero superior derecho al recibir un golpe en la cabeza que le provocó la salida del mismo de su alveolo y entonces presentó signos y síntomas característicos de una evolución desfavorable. CONCLUSIÓN: se comprobó la existencia de una reabsorción radicular inflamatoria avanzada, asociada a otros factores como: prolongado tiempo extraoral del diente reimplantado, deficiente medio de conservación, inadecuado tipo y tiempo de ferulización y el tórpido manejo endodóntico. La rehabilitación protésica inmediata fue el tratamiento adecuado para restablecer los parámetros psíquicos, estéticos y funcionales demandados por el paciente.


BACKGROUND: dental avulsion is the complete loss of a tooth from its alveolus; the best treatment for a child is the reimplantation of the own tooth but the therapeutics presents great challenges for the professionals that depend on many factors. OBJECTIVE: to demonstrate that the rehabilitation by means of immediate prosthesis is an alternative to the failure of a dental reimplantation. CLINICAL CASE: a twelve-year-old male patient that came with his father to the stomatological consultation of prosthesis of the policlinic for veterans of the medical service of the Ministry of Internal Affairs of Camagüey. Two and a half years ago, the patient underwent the reimplantation of the right anterosuperior tooth after receiving a blow to the head that caused the loss from the alveolus; then, the patient presented signs and symptoms characteristic of an unfavourable progress. CONCLUSIONS: the existence of an advanced inflammatory radicular reabsorption was confirmed, associated to other factors like: a prolonged extraoral time of the reimplanted tooth, a deficient conservation medium, inadequate type and time of splinting and the torpid endodontic handling. The immediate prosthetic rehabilitation was the suitable treatment to re-establish the psychic, aesthetic, and functional parameters required by the patient.

ABSTRACT

Objetivo: presentar el caso de un paciente con fractura dentaria de etiología parafuncional y su resolución implantoprotética. Caso clínico: una paciente de 65 años de edad, con fractura verticalc ompleta en la pieza dentaria 2.4, consultó por dolor al masticar. Se realizó la exodoncia atraumática del diente y se colocó un implante con corona provisoria, de forma inmediata. Finalmente, se colocaron un emergente y una corona libres de metal y se instaló una placa de protección neuromiorrelajante. Conclusión: la prevención y el diagnóstico tempranos de las fracturas dentarias atribuidas a parafunción, permiten la realización de tratamientos menos invasivos que conservan las piezas dentarias naturales del paciente

ABSTRACT

El protocolo de colocación inmediata de implantes post exodoncia, introducido en la década de los 90, fue descrito y propuesto como un gran aporte a la implantología contemporánea. Entre sus principales indicaciones se destacaban la necesidad de preserva el tejido duro y blando, previniendo así la pérdida ósea de la apófisis alveolar. Si bien estudios prospectivos y retrospectivos han mostrado tasas aceptables de supervivencia de implantes instalados bajo este protocolo, reciente revisiones sistematizadas randomizadas con alto cumplimineto de los niveles de evidencia, demuestran con claridad y significancia que este protocolo se asocia a un mayor riesgo de complicaciones estéticas y compromiso de la predictibilidad de la osteointegración. El objetivo del presente artículo es exponer de manera resumida, sobre la base de la evidencia científica actualmente disponible y de la descripción de un caso clínico, antecedentes que permitan aclarar ciertos mitos y controversias de un tema de gran actualidad y permanente discusión en los más importantes foros académicos, proporcionando al clínico elementos adicionales en la toma de decisiones. No obstante lo anterior, sobre la base de la evidenciay experiencia clínica documentada, resultaría razonable concluir que el implante inmediato no prevendría la pérdida ósea vertical ni horizontal y que su indicación, así como los resultados en el corto, mediano y largo plazo, serían fenómeno multifactorial que estaría fuertemente determinado, entre otros, por el biotipo periodontal, grosor de la tabla vestibular, posición del implante, manejo del gap, hábito tabáquico, enfermedad periodontal, trauma quirúrgico, presencia y características de un defecto óseo en la zona a implantar.

ABSTRACT

Este estudo teve por objetivo avaliar a estabilidade de implantes de carga imediata ou precoce, em diferentes intervalos de tempo. Material e Métodos: foram extraídos os pré-molares mandibulares, bilateralmente, de oito cães e, após 12 semanas, cada cão recebeu seis implantes (três em cada lado da mandíbula). A carga foi aplicada em pares, em um implante de cada lado, sendo que o primeiro par recebeu carga imediata, o segundo par após sete dias, e o terceiro par após 14 dias. Em cada período, as medidas de estabilidade dos implantes foram realizadas por frequência de ressonância (ISQ). Após 12 semanas, uma nova leitura da estabilidade dos implantes foi realizada. A análise estatística foi feita usando modelo linear de efeitos mistos, que é uma generalização do modelo linear padrão (Anova). Diferenças foram consideradas significantes quando p < 0,05. Resultados: a estabilidade do implante mudou no decorrer do tempo para todos os protocolos de carga, apresentando aumento significativo na estabilidade final, mas as variações foram semelhantes para os três grupos estudados. Nos períodos de sete dias e 14 semanas, o protocolo de carga imediata apresentou menor estabilidade de implante do que os protocolos de sete e 14 dias. Conclusão: a estabilidade do implante diminui nos períodos iniciais após a cirurgia e aumenta no período de 14 semanas. A estabilidade dos implantes do protocolo de carga imediata é menor do que a dos protocolos de sete e 14 dias, sugerindo cautela para seu uso...


This study aimed to evaluate implant stability in function with immediate or early loading protocols at different time intervals. Material and Methods: The mandibular premolars of eight dogs were pulled bilaterally. After 12 weeks, each dog received 6 implants (3 at each mandibular side). Implant loading was performed for each implant pair according to the following periods: immediate (fi rst pair), 7 days later (second pair), 14 days later (third pair). Implant stability measurements were performed by means of resonance frequency for each period. After 12 weeks, new readings of implant stability were obtained. The statistical analysis was performed using a mixed linear model, which is a generalization of the standard linear model (Anova), with differences considered signifi cant at p<0.05. Results: Implant stability changed throughout time for all loading protocols, presenting a significant increase in fi nal stability values, but variations were similar for the three studied groups. At 7- and 14-week periods, immediate loading protocol presented lower implant stability than 7-day or 14-day protocols. Conclusions: Implant stability diminishes in the initial periods after surgery, and increases in a 14-week period. In the middle-term, there were no statistically significant differences in the stability of immediately-loaded implants, one or two weeks after implant placement...

ABSTRACT

Introduction: In patients with hopeless prognosis of the anterior teeth there is still a strong desire to save them for the sake of esthetics. If not grossly carious, broken down or discolored the extracted tooth after suitable modifications can be placed back in its original site by splinting it to the adjacent stable teeth. Materials and Methods: Fifteen patients (10 males, 5 females) in the age range of 40-65 years with pathologically migrated, unsalvageable teeth were treated by splinting the extracted teeth immediately with the stable adjacent teeth. After 12 weeks, all the patients were explained various treatment options available for replacement of the lost teeth. The patients were asked to fill out a simple closed ended questionnaire citing the various difficulties encountered during this transitional period, selection of further treatment modalities and the reasons for their choice. The feedback obtained was then analyzed statistically. Results: Hundred percent of patients were happy with the esthetics; however, 60% of them were dissatisfied with the function that it provided. The primary problem being apprehension of splint fracture and difficulty while incising and the data was found to be statistically significant (P=0.01**). All patients demanded a permanent treatment option following this with a fixed prosthesis. None of the patients were interested in the implant supported prosthesis due to cost, treatment time involved, and need for surgery. Conclusion: The concept of immediate pontic placement is surely a viable treatment option and promises an excellent transient esthetic solution for a lost tooth as well as enables good preparation of the extraction site for future prosthetic replacement.

ABSTRACT

<p><b>OBJECTIVE</b>The feasibility and clinical effects of hollow obturator prosthesis for the repair of maxillofacial defect immediately after maxillectomy for cancer were assessed.</p><p><b>METHODS</b>Thirteen patients with T3-4aN0M0 maxillary neoplasm were treated by the prostheses immediately after maxillectomy. According to the 3D-CT reconstruction of nasal sinus, the 3D stereoscopic prototype was constructed before the surgery. Simulating surgery with Surgicare 5.0 software and then the prosthesis 3D stereoscopic model was shaped. The prosthesis was made quickly and precisely with methacrylate resins according to the model and the print mold before surgery, with supplementary tooth at the bottom of prosthesis. In the surgery, the prosthesis was installed instantly after maxillectomy. The patients were followed up at 1, 3 and 6 month after the surgery, respectively. The facial features and the pronunciation clarity were examined and the questionnaires were carried out in the patients, with comparation by paired t-test. The hollow obturator prosthesis would be replaced by permanent prosthesis made of methacrylate resins at 6 month after the surgery.</p><p><b>RESULTS</b>The hollow obturator prostheses were installed accurately and maxillofacial defects were repaired immediately after maxillectomy in the 13 patients. Postoperative follow-up showed there were significant differences in eyeball sagging (t = 4.67, P < 0.05), mid-facial region collapse (t = 5.67, P < 0.05), and pronunciation clarity (t = 16.38, P < 0.05) between patients with and without prostheses. Questionnaires showed that all the patients were satisfied with the retention of prostheses, the improvement of appearance, the improvement of the symptom of water choking and speech definition. Six months after the surgery, the hollow obturator prostheses were replaced smoothly by permanent prostheses in 11 of the 13 patients.</p><p><b>CONCLUSION</b>The precise and instant repair of maxillofacial defect by prosthesis after maxillectomy can improve survival quality of patient.</p>

ABSTRACT

This article describes the immediate placement and loading of implants in the aesthetic zone using an implant-retained, fixed prosthesis with a modified design. One section of the implant prosthesis has cemented crowns and the other section is the conventional screw-retained. This combined approach significantly offsets the unsuitable implant position, alignment or angulation, while ensuring the easy retrievability, repair and maintenance of the prosthesis at the same time.

ABSTRACT

Se presentarán los resultados de un estudio clínico multicéntrico de tres centros clínicos que realizaron prótesis inmediata individual sobre implantes en sector anterior y posterior, con una evolución de 7 años (5 años promedio), con un número de 100 casos clínicos con implantes de diferentes marcas. El 63 por ciento de la población es de sexo femenino y el 37 por ciento de sexo masculino (Fig. 1), con una edad promedio de 50 años de funcionamiento de +/- 5 años. La incidencia de tabaquismo es del 27 por ciento de los casos, todos ellos fumadores de hasta 10 cigarrillos (Fig. 2). Se evaluarán las distintas variables clínicas de aplicación, así como también las modificaciones tecnológicas actuales.

Objetivo: Avaliar após um período de cinco anos a utilização de carga precoce em 24 pacientes, em clínica privada. Métodos: Foram realizadas 103 fixações e reabilitação protética em 24 pacientes, com média de idade de 38 anos (mínima de 23 e máxima de 72 anos), sendo onze do gênero feminino e treze do gênero masculino nos quais foram efetuadas respectivamente 50 e 53 fixações. Resultados: O sucesso obtido entre prótese unitária e de prótese esplintada sobre implante foi semelhante ao redor de 92%. Conclusão: A previsibilidade entre protocolo de dois estágios distintos e o protocolo de carga imediata ou precoce funcional é semelhante; a estabilidade inicial do implante desempenha papel fundamental no protocolo de carga imediata ou precoce funcional.


Objective: After a period of five years, evaluate the use of early load in 24 patients at a private clinic. Methods: One hundred and three fixations and prosthetic rehabilitations were performed in 24 patients, with a mean age of 38 years (minimum of 23 and maximum of 72 years), eleven being women and 13 men, in whom 50 and 53 fixations respectively, were performed. Results: The success obtained among single dentures and splinted implant supported dentures was similar, around 92%. Conclusion: The predictability between the protocol of two separate stages and the protocol of immediate or early functional load is similar; the initial stability of the implant plays a fundamental role in the immediate or early functional load protocol.

El propósito del presente trabajo es comunicar el paso a paso en la confección y utilización de una tecnología simple y versátil (clon-transfer) capaz de transferir a la etapa final en la construcción de una prótesis híbrida inmediata, todos los parámetros conseguidos en la etapa prequirúrgica, de diagnóstico clínico, en lo que respecta a dimensión vertical, oclusión céntrica, soporte labial, alineamiento tridimensional de las piezas dentarias y estética aceptada por el paciente.

Solo los estomatólogos saben cuándo, hasta dónde y cómo deben recortar el yeso, porque ellos son los que han investigado y determinado la condición ósea, el grado de reabsorción de la cresta y hueso maxilar, así como la profundidad de la bolsa gingival

ABSTRACT

El análisis prospectivo hasta 2 años de la evolución clínica y radiográfica en la colocación de implantes individuales en el sector anterior con restauración inmediata. Los pacientes fueron evaluados clínica y radiográficamente a uno, tres y seis meses. El reporte preliminar a 6 meses demuestra que los implantes están oseointegrados. Se han medido los grados de reabsorción ósea por mesial y distal como criterio de evolución de la terapia. Después de 6 meses el cambio en los niveles de tejido óseo estadísticamente no es significativo. Los resultados de este estudio marcan un nivel de éxito del 100 por ciento hasta la actualidad, con un estado periimplantario altamente favorable y resultados estéticos muy buenos, con la ventaja de la reducción de los tiempos y pasos clínicos
